I've read and agree to the terms below.

• Vaccinations are current. Rabies, DHPP and Bordetella. I'll provide proof, including the rabies certificate the law requires, and I'll keep my dog on flea and tick prevention. Records can be sent on the records page.
• I've told John the truth about my dog. Health, medications, and any history of biting, guarding or reactivity. I'll tell him if that changes.
• I accept the ordinary risk. Dogs play, and play sometimes means a scrape, a strain or an upset stomach.
• Deposits and cancellations. Boarding is held with a 20% deposit, balance before drop-off; cancelled more than 5 days before the first night it is refunded in full, and inside 5 days it is kept. Daycare needs no deposit and is paid before drop-off.
• I won't send anything I'd be upset to lose. Bedding and toys get chewed and shared around.

I'm responsible for vet costs arising from my dog's own health, condition, or normal activity during a stay, including the first $200 of any claim on John's insurance. John may authorize emergency care up to $1,000 to stabilize my dog if he can't reach me or my emergency contact. Beyond that, my emergency contact can authorize more. John can decline or end a stay if my dog isn't safe with the other dogs, with him, or with his own dog. He'll call me and my emergency contact if that happens.

Photos

John may post photos and videos of my dog on his website and social media, including Instagram (@dogs_with_john). No names, no addresses, nothing showing where I live. I can opt out any time and he'll take them down. Uncheck this if you'd rather he didn't.
